Appointment Requirements & Services

Prepare the necessary documents before your scheduled visit

National ID Registration

  • PSA-issued Certificate of Live Birth and one (1) government-issued identification document which bears the full name, front-facing photograph, and signature or thumbmark
  • Philippine Passport or ePassport issued by the Department of Foreign Affairs (DFA)
  • Unified Multi-purpose Identification (UMID) Card issued by the Government Service Insurance System (GSIS) or Social Security System (SSS)
  • Student's License Permit or Non-Professional/Professional Driver's License issued by the Land Transportation Office (LTO)
  • PSA-issued Certificate of Live Birth/National Statistics Office (NSO)-issued Certificate of Live Birth with Birth Reference Number (BreN)
  • Barangay Certificate - Must contain full name, birthdate, father's full name, mother's maiden name, address, front facing photograph and applicant signature/thumbmark
  • Local Civil Registry Office (LCRO)-issued Certificate of Live Birth
  • PSA-issued Report of Birth
  • PSA-issued Certificate of Foundling
  • Integrated Bar of the Philippines (IBP) Identification Card
  • Professional Regulatory Commission (PRC) ID
  • Seaman's Book (Seafarer's Record Book)
  • Overseas Workers Welfare Administration (OWWA) ID
  • Senior Citizen's ID
  • SSS ID
  • Pantawid Pamilyang Pilipino Program (4Ps) ID

  • Certificate of Foundling or Certificate of Live Birth of Person with No Known Parent/s issued by the PSA
  • Municipal Form No. 102
  • Philippine Passport or ePassport issued by the Department of Foreign Affairs (DFA)
  • PSA-issued Certificate of Live Birth/National Statistics Office (NSO)-issued Certificate of Live Birth with Birth Reference Number (BreN)
  • Local Civil Registry Office (LCRO)-issued Certificate of Live Birth
  • National ID of the Parent (either Mother or Father) or Guardian of the Child
  • Parent or Guardian must accompany the child with valid ID
Bring original documents. No photocopies accepted for primary validation.

Status Inquiry & ePhilID

  • Valid Government-issued ID
  • Any previous transaction slip (if available)
  • For TRN Retrieval: Provide full name, date of birth, and sex for verification
  • Birth Certificate of the child
  • Parent or Guardian valid ID
  • Parent or Guardian must accompany the child
ePhilID is a valid proof of identity while waiting for the physical card.
Confidential per RA 10173 (Data Privacy Act).

Correction / Updating

  • Birth Certificate (for First/Last Name, Sex/DOB corrections)
  • Marriage Certificate (if applicable for name correction)
  • Barangay Certificate + Proof of Billing (utility bill) - for Address updating
  • PSA Birth Certificate (original) - for Sex/DOB correction
  • Birth Certificate issued by PSA or LCRO
  • Parent or Guardian valid ID
  • Parent or Guardian must accompany the child
Bring ORIGINAL copies of supporting documents for any demographic change.

National ID Appointment Guidelines

  • Strictly by appointment – walk-ins accommodated only during off-peak hours.
  • Arrive 30 minutes before schedule with printed appointment slip or reference number.
  • Wear a face mask (optional but encouraged) and practice physical distancing.
  • Minors must be accompanied by a parent/legal guardian with valid ID.
  • ePhilID printing and TRN retrieval are free of charge.
  • For lost transaction slip, present any valid government ID for verification.
